Description of student attitude, performance, and effort:
Student struggles to complete classwork within the time period provided. When student works during class to complete the work they need constant prompting to continue moving forward. Work submitted, while ‘completed’ does not appear to reflect what parents/guardians think student is most capable of. |
Rating Scale:
0 = zero help to student
4 = above expected growth/change |
|
Interventions previously applied:
- Allow natural consequences to occur.
- Communicate with the student’s parents/cottage staff (notes, emails, phone calls, etc.) to share information concerning student’s progress.
- Assign one task/assignment at a time. Give student ample time to complete it.
- Supervise the student during class assignments to maintain on-task behavior.
- Establish classroom rules and review often. Reinforce students for following the rules.

Suggestions for additional interventions:
Teacher provided:
- Specify exactly what is to be done for the completion of an assignment or task (e.g. indicate definite starting and stopping points, indicate the minimum number of requirements, etc.).
- Establish a timeline for completing a project. Expect the student to meet each deadline in order to complete the project on time.
- Organize assignments by dividing them into small segments. Set deadlines and provide the student with a reward after completing each segment of the assignment.

Student directed:
- Have student develop a checklist/chart to follow which will allow them to complete all assignments.
- Have student question any directions, explanations, and instructions they do not understand.
- Require student to begin each assignment or task within a specified period of time (e.g. three minutes, five minutes, etc.)
- Have student time assignments or tasks to monitor their own behavior and accept time limits.
- Make certain student is attending to the teacher when directions are given (e.g. making eye contact, hands free of writing materials, looking at assignments, etc.)
